Forms & surveys
Overview
Forms your website visitors fill in, and surveys you send your customers after a job. A form is built from questions — short and long text, one choice or several, a date, a number, a score, stars, a file, a consent tick and hidden fields — in sections, with questions shown only after a given answer, in up to eight languages. Each answer can become a row in a table your app opens to forms, filling only the columns the app names, or simply be kept. Adminium draws every page a visitor sees and checks every answer: no code of the add-on runs for a visitor, and nothing is fetched from anywhere else. A form is shared by its link, a QR code, a few lines for your own website (once that website is listed under Your websites) or an e-mail. Hourly limits stop a bot filling a form a hundred times, and you are told when one is reached. A consent tick asks for the newsletter or for surveys, and by default the yes counts only after a click in a confirmation e-mail. A survey is sent by the Send a survey step of an automation rule, to one person with a link of their own, and its results are added up on the survey. Answers are kept for 24 months unless you choose another time. Files a visitor sends (PDF, JPG, PNG or text) are opened by staff only. In the Sales CRM, a website form makes an enquiry, and a survey after sign-off can record the customer’s feedback — a rule you switch on. It needs Adminium 0.3.41 or newer (0.3.42 is recommended) on a public https address that visitors can reach; Studio → Privacy & public pages says whether they can.
What it attaches to
This one is not tied to one app. It works inside Adminium itself, and in any app that hosts add-ons.
What connecting lets it do
Switched on, it brings tables of its own for forms, answers, files, invitations and their mails, and its settings. Answers are written by Adminium alone: staff read and delete them, and nobody types one in. An answer fills a row only in a table an app opened to forms, and only in the columns the app named. Two roles come with it: Forms manager builds forms and surveys, reads, exports and deletes answers, and sets how long they are kept; Forms reader reads forms and answers. Both read answers with the person’s details, including the network address, browser and website an answer came from, which are kept as evidence of consent. Files a visitor sent are opened by staff only. It reaches nothing outside your installation: there is no key to keep and nothing to sign in to.

What it adds
Where it appears 4
- In Adminium: Forms in the Library — every form and survey as cards or a list, each with its questions, how to share it, and its answers or results
- For visitors: each live form on a page of its own, opened by its link or QR code, or shown inside your website once it is listed under Studio → Privacy & public pages → Your websites
- In Automations: the Send a survey step, which sends one of your surveys to a person with a link of their own, and a Survey tab on the record it was about
- In an app that opens a table to forms: an answer becomes a row there, filling only the columns the app names

If you switch it off
Disconnecting removes an add-on’s surfaces. It never drops the tables it created, deletes the files it wrote, or removes the rows it added — everything it wrote stays in your database.
What stays
Every table it created stays in your database with its rows: forms and surveys, answers, files, invitations and their mails. Rows that answers made in your own tables stay as they are.
What goes
Its Forms page leaves the dashboard. Its forms and surveys stop taking answers: a link, a QR code or a form on your website no longer opens. Rules can no longer send a survey.
About the demo
This one calls no outside service at all — in the demo or self-hosted — so it has no stand-in transport, and there is nothing to switch off. The add-ons that do reach out ship one and it is on by default, so no demo on this site makes a real call to anyone else’s service.
How to switch it on None of the demos on this site include it. In your own Adminium (0.3.41 or newer), add Forms & surveys under Add-ons and its Forms page appears in the Library. The Sales CRM’s sample data brings sample forms, closed until you open them.
